For operational activities such as quality assurance, accreditation, compliance monitoring, staff training, audits, and internal management.

As Required by Law

When disclosure is required by federal or Florida law, regulatory authorities, accreditation organizations, or legal processes.

With Your Authorization

Any other use or disclosure of PHI not described above will be made only with your written authorization, unless otherwise permitted by law.
